Botulinum toxin and dermal filler are prescription medical products everywhere in the United States, California included. Someone with prescribing authority has to evaluate you and order the treatment before a needle goes anywhere near your face. Which licence types may then perform it — physician, physician assistant, nurse practitioner, or registered nurse working under delegation — and how closely that person has to be supervised is set by the Medical Board of California together with the California board of nursing. It is one of the few things about a med spa that genuinely changes when you cross a state line.

Under the classification most widely used nationally, California is not a full-practice-authority state for nurse practitioners — an NP here practises within a collaborative or supervisory relationship with a physician rather than independently. For a med spa that has a concrete consequence: there should be a real, identifiable physician standing behind the practice, and the evaluation that precedes your treatment should be happening inside that relationship rather than around it. Ask who that physician is, how to reach them, and how the pre-treatment exam is actually performed. Classifications change; confirm the current position with the California board of nursing.

Two details vary more than anything else, and both are fair to ask about out loud. The first is supervision: some states expect the delegating or supervising physician to be physically on the premises while treatment happens, others accept that they be reachable and available, and the distinction is invisible from the waiting room. The second is lasers and energy devices, where the rules are markedly less uniform than they are for injectables — depending on the jurisdiction, a device may be restricted to licensed medical professionals, permitted to a trained technician working under supervision, or barely addressed at all.
